Reporting of an Accident Not at a Level Crossing
Form NI2508RA should be used for reporting the following:
a) all fatalities, other than those at a level crossing;
b) all dangerous occurrences, other than those at a level crossing, and including general dangerous occurrences on railway premises;
c) all major injuries to employees, other than those as a result of a dangerous occurrence at a level crossing;
d) all reportable injuries to people not at work, i.e. a person not at work taken from the site of the accident to a hospital for treatment in respect of that injury, other than those as a result of a dangerous occurrence at a level crossing;
e) injuries to people at work resulting in incapacity for work for more than 3 consecutive days.

Monthly Report of Dangerous Occurences
Form NI2508RC should be used for bulk reporting. A general exemption will be issued to railways, unless otherwise stated, covering the five dangerous occurrences listed on the form. The exemption will allow operators of railways to provide detailed monthly returns rather than immediate notification of these events (unless they are also connected with an accident to a person).
