Lock Out Procedures
Workers have lost their lives as a result of being crushed in industrial equipment. Had effective lock-out and tag practices been in place and followed, these deaths could have been prevented. Workplaces should have a written lock-out procedure for each machine outlining who locks out, the training required to perform the task safely, when lock-out is required and the steps for locking out. The purpose of lock-out is to prevent injuries caused by the unexpected start-up or movement of a machine. These procedures are to be followed every time a machine is to be cleaned, maintained, adjusted or repaired.

Liquefied Petroleum Gas - Safety Pays
LPG (normally sold as butane or propane) is supplied as a liquid under pressure and subsequently vapourised for use as a fuel. The Control of Hazardous Substances - Safety Pays
Hazardous substances are not only used or produced in factories, they can be found in offices, shops, hotels etc.. They may be used directly in the workplace like paints and cleaning agents eg bleach, or they may arise from the work process itself in the forms of fumes and waste products.
